February 16: Trump’s Historic Medicaid Shift Goes Beyond Work Requirements

Requiring able-bodied adults to work for their Medicaid is just part of the Trump administration’s drive to remake the decades-old health insurance program for the poor.

The administration signaled late last year that it welcomes state-based ideas to retool Medicaid and “help individuals live up to their highest potential.” At least 10 states have requested waivers that would allow them to impose work requirements and other obligations.

For example: They would require more recipients to contribute small monthly premiums. They would insist on monthly paperwork. They would impose lifetime limits on coverage. And they would kick recipients off Medicaid for a period of time — 30 days, or perhaps six months — for failing to follow the rules.

All the states requesting Medicaid work requirements are led by Republican governors who tout the “it’s a trampoline, not a hammock” approach to the social services safety net.

President Donald Trump looked to strengthen his administration’s plans to add work requirements to welfare programs, including Medicaid, in an executive order signed April 10.

On Monday, the Centers for Medicare & Medicaid Services rejected a proposal from Kansas to place a three-year lifetime cap on some adult Medicaid enrollees. Since Medicaid began in 1965, no state has restricted how long beneficiaries could remain in the entitlement program.

"We seek to create a pathway out of poverty, but we also understand that people's circumstances change, and we must ensure that our programs are sustainable and available to them when they need and qualify for them," CMS Administrator Seema Verma said Monday at an American Hospital Association meeting in Washington, D.C.

May 9: Work Requirements for Medicaid Are Now OK in Four States

The Centers for Medicare and Medicaid Services sent a letter to the state this week granting its approval of the state’s proposal to require most working-age, able-bodied adults to work, go to school, get job training or participate in community service for 100 hours a month in order to continue receiving health insurance under the state’s expanded Medicaid program.

Arkansas, Indiana and Kentucky also have received approval to move forward with work requirements under the program, according to the Centers for Medicare and Medicaid Services. Waivers are pending in Arizona, Kansas, Maine, Mississippi, North Carolina, Ohio, Utah and Wisconsin. Most are states with Republican legislatures and governors.

Paid work can help provide family resources, but low-wage and part-time work may not be sufficient to overcome poverty. Most working Medicaid enrollees are working full-time for the full year and are working in low-wage service jobs with limited benefits such as sick time or health coverage. Most Medicaid workers (78%) are paid hourly, and 36% of these hourly workers earn a wage at or below $10/hour. Even when working, adults with Medicaid face high rates of financial insecurity. In non-expansion states with low eligibility levels for adults, working part time at minimum wage would lead to loss of Medicaid without access to coverage through the ACA marketplaces.
